I acknowledge that I have been fully informed of the inherent risks associated with receiving a tattoo, including but not limited to scarring, infection, difficulties detecting future skin abnormalities, and allergic reactions to pigments, gloves, or aftercare products. I understand that proper aftercare is essential to reduce these risks.

I confirm that I have truthfully disclosed all known medical conditions, medications, allergies, or skin issues, and that I am not under the influence of drugs or alcohol. I understand that tattoos may affect future medical evaluations of my skin, including melanoma detection.

I voluntarily choose to receive a tattoo from Ali Dundar or Ceren Karakus Dundar (“the artist”) and I fully accept all risks associated with this procedure. In consideration of being tattooed, I hereby waive, release, and forever discharge the artist from any and all claims, demands, damages, or legal actions arising from the tattoo procedure, aftercare, or healing process. I agree to indemnify and hold the artist harmless from all liabilities, costs, or attorney’s fees resulting from my decision to be tattooed.

I understand that all deposits are non-refundable, that tattoo results vary by individual, and that no guarantee or warranty is made regarding the final appearance of the tattoo.

I consent to being photographed for the artist’s portfolio and social media, unless I state otherwise in writing.

By signing below, I certify that I am at least 18 years old, have provided valid identification, have read and understood this agreement, and voluntarily accept all terms.
